Documents needed – Outside EU/EEA

Here’s a list of every document you need to send with your application.
  • Application form, signed and dated
  • A declaration of good health and character form
  • References x2
  • A certified copy of your passport or national identity card
  • A certified copy of your diploma(s) relevant to your application
  • A certified copy of a police clearance certificate issued in your country of training and any other country where you have resided for more than six months. Please have this translated if it is not in English. 
  • £110 application fee 
  • Marriage certificate or name change certificate (if required). Please have this translated if it is not in English. 
  • A certificate of professional status in line with article 23(1) of EU Directive 2005/36/EC
  • An academic transcript of training that includes an hourly breakdown of your theoretical and practical/clinical training
  • Certificate of current professional status verifying the academic level of your training in line with article 11 of EU Directive 2005/36/EC
  • Irish applicants must provide their An Bord Altranais registration certificate
